Vial of L.I.F.E.
(Lifesaving Information For Emergencies)
The Vial of Life is a program that allows individuals to have their complete medical information ready in a known location in their home for emergency personnel to reference during an emergency if the patient is unable to speak or remember the information on their own.
The Vial of Life kit is FREE for any resident within the Crowley Fire Department response area. It comes with a sturdy magnetic vinyl pocket that holds your Medical Information form (included in the kit), DNR paperwork, insurance information, and more. The front features a clear oversized card holder to place a picture of yourself so emergency personnel can easily verify the information goes to the correct patient. You’ll also receive a 3” x 3” sticker to place on your front door or window to alert medical personnel to the location of your Vial of Life (although we strongly encourage it to be placed on the front of your refrigerator door).
